Methane SIBO (IMO): Causes, Symptoms, Testing and Treatment

In short: “methane SIBO” is now called intestinal methanogen overgrowthIntestinal methanogen overgrowth (IMO) Overgrowth of methane-producing archaea, mainly Methanobrevibacter smithii. It is diagnosed by a breath methane level of 10 ppm or more and is linked to constipation. (IMO). It is caused by methane-producing microbes called archaeaArchaea Single-celled microorganisms that are distinct from bacteria. Methane-producing archaea (methanogens) are responsible for intestinal methanogen overgrowth., mainly Methanobrevibacter smithii, rather than bacteria13. Methane slows the gut down, so IMO is strongly linked to constipation and bloating45. It is diagnosed when breath methane reaches 10 ppmParts per million (ppm) The unit breath-test laboratories use to report the concentration of hydrogen, methane and hydrogen sulfide in a breath sample. or more at any point of a breath test2, and it is usually treated with a combination of two antibiotics, rifaximinRifaximin An antibiotic that is barely absorbed into the bloodstream, so it acts mainly inside the gut. It is a common prescription treatment for SIBO. and neomycin17.

What is methane SIBO (IMO)?

Breath testing has shown that what used to be called SIBO is really at least three different overgrowth patterns, each with its own microbes: hydrogen-dominant SIBO, intestinal methanogenMethanogens Archaea that produce methane from hydrogen and carbon dioxide. Methane slows the movement of the intestines. overgrowth (IMO) and intestinal sulfide overproductionIntestinal sulfide overproduction (ISO) Excess hydrogen sulfide made by sulfate-reducing bacteria. It is linked to diarrhea and can only be detected by three-gas breath tests. (ISO)1. The methane pattern was renamed because the organisms responsible are not bacteria at all. They are archaea, a separate branch of life, and the main species in the human gut is Methanobrevibacter smithii13.

Methanogens live on hydrogen made by other gut microbes and turn it into methane1, so methane production depends on the bacteria around them. For the hydrogen and sulfide patterns, see what SIBO is and SIBO symptoms by gas type.

Symptoms: why methane causes constipation

Methane is not an inert gas. In animal experiments, infusing methane into the small intestine slowed transit by an average of 59%, and methane-producing patients with IBS showed different small-bowel contraction patterns from hydrogen producers6. In people, the link with constipation is consistent:

  • A meta-analysis of 9 studies with 1,277 people found that those with methane on a breath test were 3.5 times more likely to be constipated4.
  • A 2025 meta-analysis of 19 studies found that people with IMO most often reported bloating (78%), abdominal pain (65%), flatulence (56%) and constipation (51%). Compared with people without IMO, they had more frequent and more severe constipation and less diarrhea5.

IMO is associated with constipation-predominant irritable bowel syndrome (IBS-C), whereas hydrogen-dominant SIBO is associated with the diarrhea-predominant form (IBS-D)13. Read more about the overlap in our guide to IBS and SIBO.

How methane SIBO is diagnosed

IMO is diagnosed with a hydrogen and methane breath test. Under the North American Consensus, a methane level of 10 parts per million (ppm) or more at any point during the test counts as positive2. Unlike the hydrogen criterion, which looks for a rise within the first 90 minutes, the methane criterion does not depend on timing. You can check your own report against this threshold with our breath test interpreter.

Treatment options

Because methanogens are archaea, antibiotics designed for bacteria do not work as well against them1. The studies so far are small, and larger randomized trials are still needed3.

Approach What was studied Result
Rifaximin + neomycin Rifaximin 400 mg three times daily plus neomycin 500 mg twice daily for 10 days (retrospective review of 74 patients) Methane cleared in 87% on the combination, compared with 28% on rifaximin alone and 33% on neomycin alone; 85% improved clinically7. A double-blind study also found the combination better than neomycin alone1.
Elemental diet 2 weeks of an exclusive palatable elemental formula in 30 adults with SIBO and/or IMO Peak methane fell from 41 to 12 ppm on average, 73% normalized their breath test and *M. smithii* levels dropped8.
Adding herbs and probiotics Herbal antimicrobials, probiotics and prebiotics added to antibiotics and a low-FODMAP diet (179 patients) No further drop in gas levels, but higher symptom remission, especially in methane-dominant cases9.

Herbal approaches for methane, such as allicin from garlic, are popular, but they have not been tested on their own in human trials; see what the evidence shows for herbal antimicrobials. For a side-by-side overview of treatment types, see our treatment comparison.

Diet with methane SIBO

A low-fermentation diet can reduce symptoms, but with IMO it should not cut fiber so far that constipation gets worse. A 2025 review recommends choosing low-fermentation, fiber-containing fruits and vegetables such as citrus fruits, berries, leafy greens, root vegetables and zucchini10. See our SIBO diet guide for the full approach.

Preventing it from coming back

As with other types of SIBO, clearing the overgrowth does not fix the reason it developed. Because IMO is tied to slow transit, supporting the migrating motor complex and addressing root causes are central to preventing relapse.
